UI Records Retention Schedules
Please note: The official University of Iowa records retention schedules are under revision. Updates will be posted periodically.
These retention schedules are designed to assist university departments in preserving official records appropriately while providing economy and efficiency in the creation, organization, maintenance, use, and disposition of records.
Search Criteria
Administrative unit: represents either the administrative unit responsible for establishing the official record retention criteria OR the administrative unit responsible for maintaining the official records identified.
Confidential: indicates official records that contain information of various sorts that the university receives and holds confidential unless otherwise ordered by a court, by the lawful custodian of the records or by another person duly authorized to release such information. Examples include: student records, medical records, personnel records, etc.
Vital: indicates official records that contain information considered essential for the operations of a department (and/or the university) and includes information that may prevent a department from incurring serious liability or risk, or that would be extremely costly to replace. In the event of a disaster, this information, if destroyed, would make it difficult for a department to conduct normal business activities.

Unit or program reviews and unit responses conducted as required by UI Policy Manual (formerly Operations Manual), to allow the university community to examine operations and programs and evaluate how well they function in relation to the university's mission as well as university guidelines, policies, and procedures.
Departmental, Unit, Collegiate, Program, Accreditation Self Studies
Unit or program self-studies conducted as required by UI Operations Manual policy, to allow the university community to examine operations and programs and evaluate how well they function in relation to the university's mission as well as university guidelines, policies, and procedures.
Diagnostic and Clinical Division Correspondence, Forms, SOPs
Compliance monitoring data should be made legally defensible by keeping thorough and accurate records. The Quality Assurance (QA) plan and/or Standard Operating Procedures (SOPs) need to describe the policies and procedures used by the facility for record integrity, retention, and storage (EPA order 5360.1). Be able to reconstruct all testing with data.
Diagnostic and Clinical Division Test Results, Analytical Data, Sample Submission Records
Per 2013 Administration (AD) Iowa Public Health Standards: “STANDARD AD 1: Maintain written policy and procedure to assure privacy and security of public health records in accordance with the applicable state and federal regulations (HIPAA, client bill of rights, computer policy, clinical records, secure document storage, and record retention).”
